Also required to be sent back with this form are the following:

• Certified copies in English of the applicant's most recent school reports
• A certified copy in English of a testimonial from the Principal of the applicant's present school or school previously attended
• Four (4) copies of the applicant's passport photograph

Please provide English translations of documents as well as copies in applicant's language of origin.
